The purpose of this study is to introduce a novel approach that is reliable in predicting severities of the maize common rust disease by CNN deep learning models. This was achieved by applying threshold-segmentation on images of diseased maize leaves (Common Rust disease) to extract the percentage of the diseased leaf area which was then used to derive fuzzy decision rules for the assignment of Common Rust images to their severity classes. The four severity classes were then used to train a VGG-16 network in order to automatically classify the test images of the Common Rust disease according to their classes of severity. In the absence of these guidelines the professional must adhere to principles of good practice.1 Guidelines provided by the law aim to standardize medical acts. They are based on the principles of evidence-based medicine, an approach to medical practice aimed at optimizing the decision-making process. In this article a historical perspective is proposed, finding analogies between the principles of the “Gelli” law and positivist thought. The evolution that Positivism has had in the medical field has been analyzed, in particular studying the evolution of the thought of the Italian physician and strong positivist Paolo Mantegazza (1831-1910). In light of the facts it is concluded that the physician must always keep in mind that medicine is a science applied to man, and any medical act must be a consequence of a doctor-patient concordance that inevitably takes into consideration every single individual in his uniqueness. We present the cases of two adult male patients with painfully delayed union of proximal tibia diaphyseal fracture after intramedullary nailing. Patients underwent to nail dynamization and Poller blocking screw augmentation at 3 and 5 month, respectively, after the index surgery. Both patients were pain-free after the surgery and bone-union were radiographically evident after 3 months. At 12-month follow-up, patients returned to their previous activities.

Although nail dynamization is the choice treatment to obtain fracture compression for delayed union of tibial shaft fractures, augmentation with Poller blocking screw could provide additional mechanical stiffness in unstable supra-isthmic shaft fractures of the tibia and favour bone union.
